  • Bring your insurance information and prescription or referral.
  • Photo ID
  • Please wear comfortable clothing that you’d normally wear to exercise in.
  • Appointments last approximately 60- 90 minutes.
  • For billing questions please contact Denise at 253-732-1642.
  • Our therapists are IN NETWORK for most of the local and national insurance plans. We also accept and are able to bill any insurance as OUT of NETWORK. Each patient or Guardian is responsible to verify and understand their individual insurance benefits and requirementsincluding referral requirements, annual deductables, copay and/or co-insurance anounts. Call the customer service number on your insurance card and ask about “outpatient physical therapy” benefits.

Please give at least 24 hours notice if you need to cancel an appointment. (253) 862-2575.

Please arrive 15 minutes prior to your scheduled appointment time to allow time to complete your paperwork. The receptionist will be available to assist you at any time.

Hand therapists are specialized in treating upper extremity conditions including:

  • Insurance card.
  • Photo ID
  • Physician referral.
  • Applicable co-pay or deductible.
  • L&I Claim Form (if this is a work related injury).
  • Authorization Form from your Insurance Company (if required).

On your first visit, you will be evaluated by one of our therapists and you’ll discuss your treament options. Please plan on spending 60-90 minutes for your initial evaluation.

  • Please come 15 minutes before your first appointment to fill out paperwork.
  • Wear loose fitting clothing
  • If we are evaluating your shoulder, please wear a tank top or clothing that exposes your shoulder and part of your upper back.
  • If we are evaluating your knee, please wear shorts or loose pants/sweat pants that can be pulled up above your knee.

Patient Education Resources


The Practice Standards Commission has developed Patient Education Resources for therapists to use as a convenient tool for patient care. These flyers provide information on common diagnoses, answer frequently asked questions and educate patients on the role a hand therapist plays in their treatment.
